The if-else cases in lsqr.py were not exhaustive. They become exhaustive by changing ab > aa to ab >= aa.

Put everything in namespace dionysus. Besides making things more organized, it
will also allow us to get rid of the problem of having to typedef Simplex<...>
to Smplx. If it were in dionysus namespace, then
`typedef dionysus::Simplex<...> Simplex;` would work.
